Get started
You or your child are enrolled in an Incentives Program that lets you earn rewards by doing healthy activities. You received the card because you or your child completed a healthy activity. Use this card to spend your rewards.
Call the number on the back of your card.
You can log in to the Healthy Benefits+ account through the website or app.
On the website, click Log in in the top menu. If you can't remember the password, click Forgot password?
In the app, tap Sign in with HealthSafe ID. If you can't remember the password, tap Forgot password?

Rewards are subject to change. Rewards expire one year from date of issuance. If you are no longer a CareSource member, you could lose access to the rewards portal and your rewards are no longer available. The rewards available will vary depending on your health care needs. Note that not all reward activities are covered services annually. Please check with your primary care provider (PCP) or Care Coordinator to see what services you may need.
