If enrolled in an eligible UnitedHealthcare plan, you will receive your UnitedHealthcare UCard® after your enrollment application has been approved.
To activate your card, use the UnitedHealthcare app, visit activate.uhc.com or call 1-866-757-1864, TTY 711.
